What is MTP Kit (Mifepristone+Misoprostol)?

MTP Kit contains one 200 mg Mifepristone tablet and four, 200 mcg Misoprostol tablets. This kit is effective in terminating a pregnancy when it is less than 9 weeks old. Mifepristone is taken by mouth, while Misoprostol should be inserted into the vagina.

The MTP Kit is manufactured based on the guidelines of Obstetricians and Gynecologists, thus it is safe. Nevertheless, it should be administered under the supervision of a gynecologist within a medical facility.

MTP Kit: Doses & Adminitration

The MTP kit is to be consumed in the following manner: take the 200 mg mifepristone orally, then on the 3rd day, insert the 4 misoprostol (800 mcg) tablets in the vagina. Misoprostol may be self-administered or may be administered by a professional medical professional.

Misoprostol should be able to cause abortion within 4 hours; otherwise, a second dose of 400mcg Misoprostol may be administered either orally or through the vagina.

The patient is advised to go for a follow-up visit between 7 and 14 days of administration of the MTP Kit. The importance of this visit is to confirm if the pregnancy was terminated. Confirmations are done through USG scan or clinical examinations. If the kit failed, there is a risk of fetal malformation. Thus surgical termination of the pregnancy is recommended.

Note that MTP Kit is indicated for termination of intrauterine pregnancy not exceeding 9 weeks or 63 days. Calculation of the pregnancy’s age is based on either the first day of amenorrhea or the last menstrual period. For this kit, the first day of amenorrhea is used, assuming it is a 28-day cycle, and ovulation occurs on day 14.

It is crucial that the duration of pregnancy is known, so as the kit to work effectively. This duration is determined by looking at menstrual history as well as through clinical examinations. If a precise duration cannot be established, then, ultra-sonogram (USG) scan should be done. Again, a USG can be used if ectopic pregnancy is suspected.

How does MTP Kit work?

Mifepristone has an anti-progesterone effect. While progesterone works to keep the fetus within the uterus, Mifepristone works to expel the fetus, thus terminating the pregnancy. This synthetic steroid causes the muscular wall of the uterus (myometrium and endometrium) to contract and causes the cervix to dilate. This leaves a clear way for the already detached fetus to leave the womb. Apart from anti-progesterone effects, Mifepristone bears anti-glucocorticoid as well as a weak anti-androgenic effects.

By contrast, Misoprostol softens the cervix as wells contract the uterine walls. It stimulates the myometrium to contract. The sum of all these contractions causes the fetus to be expelled. Still, Misoprostol is a synthetic prostaglandin, a potent antagonist of gastric acid secretion in humans.

Contraindications for MTP Kit

The following are contraindications associated with the use of MTP Kit:

  • Ectopic pregnancy
  • IUD in place
  • Chronic adrenal failure
  • Bleeding disorders
  • People on anticoagulant therapy/corticosteroid therapy
  • Inherited porphyria
  • Severe uncontrolled asthma

Abortion is a risky process that may require emergency treatment in case of incomplete abortion, blood transfusions, and emergency resuscitation. So people undertaking the procedure must always be near a health facility.

MTP Kit: Warnings & Precautions

The administration of MTP Kit should be done and monitored by a certified gynecologist. So people with malnutrition, renal failure, and hepatic failure should not use MTP Kit, unless authorized by a doctor.

The safety and effectiveness of MTP kit are not apparent in women suffering from hypertension, heart disease, liver dysfunction, insulin-dependent diabetes, respiratory or kidney disease, severe anemia, or heavy smoking, because no data exists. Besides, smokers above 35 years of age should be treated with great caution.

Before using an MTP Kit, the patient should be advised on its effect, importance, and the necessity of the second visit. Clinical examination is necessary to determine if complete pregnancy termination after the treatment.

Be informed that following the termination of a pregnancy, you may conceive again. So it is essential to use contraceptives as soon as you resume sexual intercourse.

Some women may experience decreased hemoglobin concentration, hematocrit, and red blood cell count due to heavy bleeding.

MTP Kit: Drug Interactions

Specific Studies about the interaction between MTP Kit and food or drugs have not been conducted. Nevertheless, it is possible that some drugs could inhibit the metabolism of Mifepristone. Such drugs include ketoconazole, erythromycin, itraconazole, erythromycin, and grapefruit. Some medicines have also been found to induce the metabolism of Mifepristone. They are rifampicin, certain anticonvulsants, and dexamethasone.

Clinical studies, on the other hand, state that Misoprostol doesn't interfere with the functioning of aspirin. Misoprostol does not have any clinical impact on the absorption a well as anti-platelet effects of aspirin. Similar studies have been carried out between Misoprostol and several NSAIDs to ascertain if there are any interactions. The conclusion made was that Misoprostol had no clinical impact on ibuprofen, diclofenac, piroxicam, aspirin, naproxen, or indomethacin.

When it comes to antacids, care should be taken. Antacids containing magnesium should be avoided while on treatment with MTP Kit because it may worsen diarrhea that Misoprostol causes.

Side Effects of MTP Kit

MTP Kit is designed to induce abortion by detaching the fetus from the uterine wall and causing the uterus to contract. This effect is accompanied by vaginal bleeding and cramping. In this case, the typical side effects that a woman can experience is abdominal pain, which is caused majorly by uterine cramping.

Typically, most women using the MTP Kit indicate that they faced a side effect. Other women indicate experiencing more than one adverse side effects, and 90 percent of women reported that the side effects usually occur after the third day of treatment with MTP Kit.

Side effects differ from woman to woman. But the majority report vomiting, diarrhea, and nausea. Headaches, dizziness, and pelvic pain are other side effects, but they rarely occur. By the 14th day after using Mifepristone and Misoprostol, the side effects will have disappeared; the only thing that is experienced is bleeding and spotting.

MTP Kit: Overdose

Currently, no severe side effects have been reported concerning Mifepristone. In fact, the dosage can be increased to threefold as a pregnancy termination measure, without the patient experiencing any effects. However, the massive ingestion of this drug leads to signs of adrenal failure. When acute intoxication occurs, special treatment may be administered. Such treatment may involve the use of dexamethasone.

For Misoprostol, the human toxic dosage, as of now, has not been determined yet. Humans tolerate a total dosage of up to 1600 mcg, but with a mild form of gastrointestinal discomforts. But when Misoprostol is taken in excessive amounts, it can lead to severe side effects like abdominal pain, convulsions, tremors, bradycardia, hypotension, sedation, breathlessness, diarrhea, and fever.

These side effects can be life-threatening. As such, you need to seek medical help immediately.
