What is Cytotec (Misoprostol)?

Cytotec (Cytolog) is used in conjunction with Mifepristone to terminate pregnancies. Usually, it’s taken 2 or 3 days after consumption of Mifepristone. Cytotec  is sold in generic form as Misoprostol.

During pregnancy termination, the patient takes Mifepristone first, which blocks the action of a hormone called Progesterone.

Progesterone is a hormone that maintains pregnancy. Mifepristone, on the other hand, widens the cervix and detaches the fetus from the uterine wall. The fetus lacks food and oxygen.

Then, the patient takes Cytotec, on the third day. The pills are placed under the tongue to melt naturally. However, the tablets can be taken with food or milk for quicker action.

Dosages and administrations of Cytotec?

The standard dosage is 1 Mifepristone pill of 200 mcg, taken on the first day. This should be taken orally with water. Then Cytotec is taken on the third day; 4 pills each 200 mcg. This makes a total of 800 mcg of Cytotec. Intake of Cytotec should cause an abortion within 3 hours; if not, a second dose is advised.

A gynecologist administers the drug; however, in some instances, it can be self-administered. A follow-up visit is essential after this treatment. The visit should be done between 7 and 14 days. Its importance is to confirm whether the abortion was successful or not. The confirmation is done through a USG scan or clinical examination. If the abortion wasn’t successful, then surgical termination or suction is advised.

Cytotec is strictly prescribed to cause abortion of a fetus not more than 10 weeks old. The pills only work in the cases of intrauterine pregnancies. It, therefore, will not work in cases of ectopic pregnancy.

The age of the pregnancy can be determined by either considering the last menstrual period or the first day of amenorrhea. Treatment with Cytotec considers the first day of amenorrhea in a cycle that takes 28 days, with ovulation occurring on the 14th day.

The gestational age is determined through medical examinations and menstrual history. Where pregnancy age is uncertain, an ultra-sonogram (USG) scan must be conducted. If you have an intrauterine device (IUD), remove it before using this drug.

Treatment with Cytotec is done in 3 steps

  1. Prescription of a single Mifeprex tablet on day one
  2. Prescription of four Cytotec pills on the third day
  3. Confirmation of the pregnancy termination was successful, usually after 2 weeks. An ultrasound exam is done to give precise results.

Cytotec causes uterine cramps and vaginal bleeding for up to 4 weeks. If you do not bleed or it is minimal, consult your doctor.
In unsuccessful circumstances, surgical pregnancy termination may be recommended.

Contraindications of Cytotec

Contraindications refers to a factor that acts as a reason for withholding a medication or a treatment because of the possibility that it could harm the  patient.

  • Migraine issues
  • IUD in place
  • Chronic adrenal failure
  • Bleeding disorders
  • Anticoagulant/ Corticosteroid therapies

Evidence exists of human fetal risk and abnormalities in studies conducted in both animals and humans. Therefore, patients must be aware of such effects, as well as the importance of visiting doctors while on this treatment.

After treatment with Cytotec, patients must initiate the use of contraceptives because another pregnancy may occur a few weeks later.

The exact age of the pregnancy must be determined before the administration of this drug. This is important for better results to be obtained.

Due to heavy vaginal bleeding, some women might experience a decreased red blood cell count, hematocrit, and hemoglobin concentration.

Cytotec usage causes pain. As such, doctors prescribe painkillers.

Patients with Rh (D)-immune globulin need initiation within 72 hours of initial dose administration.

Follow up visit is imperative to determine the complete expulsion of a fetus. In case the drug failed, a patient may either choose suction curettage or expectant management.
